Overview
Maternal mortality in Tanzania is approximately 556 per 100,000 live births. Key causes include haemorrhage, sepsis, unsafe abortion, and hypertensive disorders of pregnancy (preeclampsia/eclampsia). Preeclampsia — high blood pressure after 20 weeks of pregnancy plus protein in urine — can progress to eclampsia (seizures) and is life-threatening. All women should attend at least 8 antenatal care (ANC) visits and deliver at a facility.
Common Symptoms
Preeclampsia warning signs (after 20 weeks):
- Severe headache
- Visual disturbances (blurred vision, seeing spots)
- Upper right abdominal pain
- Sudden swelling of face, hands, and feet
- Decreased fetal movement
Eclampsia: Seizures — a medical emergency. Go to hospital immediately.
Treatment
- Preeclampsia: Magnesium sulphate (prevents seizures), antihypertensives (labetalol, nifedipine), monitoring, delivery if near term
- Eclampsia: Emergency magnesium sulphate + delivery
- Postpartum haemorrhage: Oxytocin immediately after delivery (given routinely at facilities)
- All births should occur at a facility with skilled staff
- Blood pressure screening at every ANC visit
Prevention
- Attend all 8 antenatal care visits (free at public facilities)
- Deliver at a health facility — never deliver at home alone
- Low-dose aspirin from 12 weeks reduces preeclampsia risk in high-risk women (ask your midwife)
- Manage chronic hypertension and diabetes before pregnancy
- Calcium supplementation in pregnancy (provided free at ANC)
Cost & NHIF Coverage
Public facilities: Antenatal care, blood pressure monitoring, and delivery services are free or heavily subsidised at government facilities under Tanzania's maternal health policy.
NHIF: Covers antenatal visits, delivery (including caesarean section if needed), and preeclampsia management at accredited facilities.
Private facilities: Antenatal package TZS 100,000–300,000; delivery TZS 300,000–1,500,000 depending on facility and whether a caesarean is needed; preeclampsia management requiring admission and monitoring adds further cost — confirm your insurer's maternity benefit before choosing a private facility.
