Mainland Tanzania vs Zanzibar: How the Two Health Systems Differ

Two Health Systems, One Country
Tanzania is a union of mainland Tanzania and Zanzibar (Unguja + Pemba). Health is a non-union matter — each side runs its own Ministry of Health, its own public facilities, and (until recently) its own insurance scheme.
Two Ministries, Two Networks
- Mainland — the Ministry of Health (Tanzania Mainland) runs the dispensary → health centre → district → regional → national referral hospital tier.
- Zanzibar — the Ministry of Health Zanzibar runs its own facility network across Unguja and Pemba, reporting to the Revolutionary Government of Zanzibar rather than the Union government.
Facility ownership categories (public, private, faith-based) exist on both sides, but the regulator, licensing body, and often the pharmacy authority (ZFDA on Zanzibar vs the mainland Pharmacy Council) are separate.
Insurance: NHIF vs ZHSF
- NHIF (National Health Insurance Fund) — the mainland's main public scheme, historically for formal-sector employees and dependents.
- ZHSF (Zanzibar Health Services Fund) — Zanzibar's mandatory public health insurance scheme, covering residents at Zanzibar's public and faith-based facilities.
Reciprocity since 1 July 2024: NHIF and ZHSF recognise each other's cards. An NHIF member visiting Zanzibar can use NHIF-accepting facilities there, and a ZHSF member visiting the mainland can use NHIF-accepting facilities — subject to each facility's own network agreement. Always confirm at reception, since not every facility on either side participates.
