Public vs Private Healthcare in Tanzania

Compiled from public health references including the CDC, Mayo Clinic, WebMD, the World Health Organization, and the Tanzania Ministry of Health.
The Two Systems
Tanzania has a two-tier healthcare system: government (public) facilities and private facilities. Both accept NHIF, but the experience often differs.
Public Facilities
Pros:
- Lower or zero out-of-pocket costs for NHIF members.
- Wide geographic coverage, especially in rural areas.
- National referral hospitals offer specialized care.
Cons:
- Can be crowded, with longer waiting times.
- Equipment availability varies.
Private Facilities
Pros:
- Shorter waiting times and more amenities.
- Often newer equipment.
Cons:
- Higher costs if NHIF is not accepted or for uncovered services.
- Concentrated in cities (Dar es Salaam, Arusha, Mwanza).
Which Should You Choose?
For routine care, public clinics are often sufficient. For specialist referrals or when speed matters, private hospitals may be worth the cost. Always check whether your insurer is accepted — even at private hospitals with an NHIF badge, specific services may not be covered.
